This year’s Data Verification Forms have been distributed to students today. Please:
- review the information on both sides to ensure accuracy
- make any changes required
- sign the bottom of the second page
- have your student return the form tomorrow to their first period teacher or to the main office.
Please note that any address changes will require proof of address.
