Start Up Packages for the 2017-2018 school year will be available in the front foyer from Monday, August 28 to Thursday, August 31 from 8:00 a.m. to 3:00 p.m. The Start Up Packages include important documents that must be signed and returned on the first day of school.

These include:

• Data Verification Form (or DVF)
• Release Form for Use of Student Information
• Dance Contract.

Pre-printed information on the DVF should be verified. Please write in any changes. Parent signatures on all forms are required, with the Dance Contract also requiring the student’s signature. Please keep these forms stapled together when returning them to the school.
